Booking an Appointment
To book an appointment with Dr Debo Gorai, you will need a valid referral from your General Practitioner (GP) or Specialist Physician.
Please complete our online appointment request form and upload a copy of your referral where requested. Once your referral and appointment details have been received, the Synapse Neurology reception team will review your information and contact you as soon as possible to arrange a suitable appointment time.
If your GP or specialist has already sent your referral directly to Synapse Neurology, you can still complete the online form so we have your current contact and appointment details.
New Patients
If you are a new patient, please also complete our online New Patient Registration Form before your appointment. This allows us to collect the information required for your consultation in advance and can help make your check-in process easier on the day.

Workers Compensation and TAC Patients
Please note: Workers Compensation and TAC patients will only be given an appointment once Dr Gorai’s rooms have received written approval from the insurance company. The insurance company may fax, email or post the approval letter to:

Please find the NCS / EMG referral form (PDF file) below to refer patients. Please print and Fax to 03) 8678 1034 or Email to info@synapseneurology.com.au. Once we receieve a referral, our rooms will contact the patient to make a booking as soon as possible.
